Miami has a caregiver problem, not an agency problem

You can see it in the search results. Search for home health care in Miami and one of the agencies ranking on the first page is not showing you a page about care at all. It is showing you a recruitment page, aimed at caregivers, headed with an invitation to join their team.

Two of the eight searches Google suggests alongside this one are about careers and salaries rather than services.

That is what a workforce shortage looks like from the outside. Agencies in this city compete for caregivers at least as hard as they compete for clients, and the ones losing that competition solve the problem by rotating whoever is available into whichever house needs covering.

For a family, that shows up as a different person every week. For someone with dementia, it shows up as a person who has to be introduced from scratch every visit.

Why continuity matters more than almost anything else

Care at home only works when the caregiver knows the house and the person in it. Where the towels are. Which mug. That the left hip is the bad one. That she will say she has eaten when she has not.

None of that is in a care plan. It accumulates, and it is lost every time the caregiver changes.

The practical consequences of high turnover are:

• Medication errors: Because the routine is unfamiliar.
• Falls: Because the new caregiver does not know which transfer she can manage and which she cannot.
• Refused care: Very common with dementia, and almost always about the stranger rather than the task.
• Family exhaustion: Because someone has to re-explain everything, every time.

What to ask any Miami agency before you sign

• How long do your caregivers stay?: Not the industry average. Theirs.
• What happens when the regular caregiver cannot come?: Ask whether you get a familiar backup or whoever is free.
• How many different people will come to the house in a month?: Ask for a number. Hesitation is the answer.
• Is the caregiver employed or referred?: Florida licenses two different models. A home health agency employs its caregivers and is accountable for the care. A nurse registry refers self-employed contractors. Both are legitimate, and they behave differently when someone calls in sick. • Who supervises, and how often?: A registered nurse writes and reviews every one of our Miami care plans. Aides do not work unsupervised.
• What language does the caregiver actually speak, at what level?: Not "we have Spanish speakers." Ask about the person who will be in the house.

Language, in the place it matters most

Our caregivers include Spanish and Haitian Creole speakers, and in Miami that is a practical requirement rather than a nice addition. A caregiver who cannot follow a conversation about medication with the person taking it is not much use, however kind they are.

Paying for care

Medicare covers skilled nursing and therapy after a qualifying hospital stay, and covers aide visits only alongside those skilled services. It does not fund ongoing daily personal care.
