Cleveland Clinic Martin North sits at 200 SE Hospital Avenue in Stuart. Martin South is four miles away at 2100 SE Salerno Road. Both run 24-hour emergency departments, and between them they handle the great majority of hospital admissions in the county. Cleveland Clinic Martin Health-Stuart is ranked among the top hospitals in Florida.

That concentration shapes what home care looks like here. Some families arrange care after a slow decline. Many more arrange it in the 48 hours after a discharge planner has said the words "she cannot go home alone."

If that is where you are right now, three things are worth knowing.

• The discharge date can move: It usually moves earlier, not later. Do not wait for a confirmed date before arranging care, because the confirmed date is often the day it happens.
• Ask the case manager for the discharge summary and the medication list before you leave the building: Our nurse will need both, and getting them afterwards takes days.
• Skilled nursing and home health aide services are different things with different funding: Skilled nursing after a qualifying hospital stay may be covered by Medicare. Day-to-day help with bathing, dressing, meals and supervision usually is not. Most Martin County families end up using both, funded differently. Martin County sits in Florida Medicaid Region G, which since February 2025 has covered Indian River, Martin, Okeechobee, Palm Beach and St. Lucie under a single set of managed care contracts running to 2030.
